I have a detailed specific question about the extended bolus delivery settings on the Tslim. I’ll call Tandem of course, but I’ll ask here too since I often get good answers here. I’ve RTFMed but the manual doesn’t address my specific question.
Sometimes I eat a sugary dessert and even when I deliver some of the bolus now and the rest over, say, a two hour extended duration, the dessert takes so long to even START raising my BG that I’ve already got the entire bolus in me before my BG goes through the roof. Then I have the usual hours long fight to get it back down.
I know to tell the pump “give me 25% now and 75% over an extended 2 hour duration”. [I’m making up the numbers for the sake of this question. Please DO NOT answer by commenting on the numbers at all. Right now they’re not my point.]
What I don’t know how to do is tell the pump all this AND tell the pump “and don’t START that 2 hour extended delivery for 3 hours from now”.
I’m not aware of any way you can do this, except of course manually later on, or I guess with some complicated customization of the CIQ basal feature, but that’s not really practical. For my tSlim, 2 hours out is the max to schedule delivery of a remaining split bolus without just doing a new bolus sometime after 2 hrs.
If you do call Tandem about this, I’d be curious to see your comment someday if there is a way to expand the time or otherwise further customize the extended setting
An extended bolus is useful for a high protein meal where it is possible to bolus then go low because the carbs aren’t available yet. The extended bolus is useful to stretch out to insulin. I used it extensively when I had an aucheck advantage pump 20 years ago. If the Tslim has this function I don’t use it. I now use lyumjev and a very low carb diet. Some days I don’t bolus at all. I have been T1D for 59 years and using a pump (8) for 40 years.
With the new update on Tandem x2, you can extend a bolus for much longer than 2 hours. I hope you can get the new update. As to telling the pump not to start the extended bolus until late, that is not possible.
