New pumper - odd method of establishing rates and ratios?

I have to say I had a real jerk the first endo I went to, after 2 visits I absolutely refused to go back to him, even though he was supposed to be good. If I had stuck with him I think I would have ended up very sick. I had asked if I could be a type 1 as I swam laps every day and had been a vegan for over 25 years, and I had an uncle that died from type 1. I was told no without him ever testing me. He wanted to put me back on the medications that had made me so sick and got mad at me when I changed my dosing of insulin. I was told don’t change it. It wasn’t until I switched doctors to an internist that I thrived and was then sent to a new endo that was hired and was diagnosed right.

I have to tell you I would refuse to see him again. It honestly is your daughters life and it makes no difference in the scheme of things if everyone there knows you refuse to see him. Just tell them that you were in tears and you don’t get along with him. That you are uncomfortable with getting that upset at a doctors appointment. See one of the other ones or switch endos. It makes such a huge difference when you like your doctors and endo and get along with them. I was then blessed with a new endo that was a type 1 and tested me right away. I loved her dearly and got along with her really well.

That second endo that was a type 1? She would just glance at my upload of data each visit and make suggestions that I might want to do, but never said I had to change anything or even pushy about changing anything. (I was doing well though). I valued her suggestions, especially starting on a pump and it was new stuff to me! So she knew a lot more about how the settings would work for certain things that I hadn’t thought of.

I personally like a shorter duration. As when I was on shots I knew my duration was up to 6 hours. But most of it’s effectiveness is done after 2-2 1/2 hours. So I have mine set at 2 1/2 hours, so it lets me correct faster. Of course then you do have to watch from going too low with the residual insulin action. So if you are active all day like a teen might be and in classes I would probably opt for a longer time.

Do you have a dexcom with the clarity ap? Because then you know at a glance what your A1C is for the last 2 weeks, month or 90 days. It’s pretty close to being accurate. It will let you judge better how you’re doing.

I have 10 different rates in my pump for the day right now lol, I have major dawn phenomenon and have a tons of different morning settings just for that!

This may be a good starting point, but should not be forced. Those on lower carb will tend to have higher basal %.
Many things will influence the ratio, and no need to think adjustments are required to force 50/50.

Any doctor that is berating you and your daughter is failing at job #1 - focus on the patient. Chronic conditions like diabetes require us to be the experts at us, and our doctors to be part of our support team.

This endo seems to have a “My way or the highway” attitude. I would take the highway.

I was reading your post and also wanted to cry!
I remember when a was 9 we switched doctors to see “the best” endo in the area. My parents were. Dry driven to see that I got the best care. And back than, you just didn’t question a doctor or anyone in an authority position.
And over 2 years,I would get mentally beat up. And yes, I would cry on the way home after every visit. And at one visit, I started crying before we got into the car and that was the point my Mom realized he wasn’t helping but hurting.
I am hoping and praying you can find someone else to help you in this journey. A doctor who is helping you with a chronic condition needs to be helpful and supportive and be someone who listens. It sounds like this doctor is checking none of this boxes. So get out and get out now before real mental damage an be done. It has taken me many, many years to try and overcome that one doctor.
It sounds like you all have a good understanding of what you are doing. Find someone who can listen to what you want not what they want. You’ve got this!

Thank you for your post. I agree wholeheartedly. We saw our family doctor today and got the ball rolling for a referral to an adult Endo who is well liked and supportive. We refuse to see that doctor again.
