1-hour post-prandial and correction/bolus dose questions

Thanks for the suggestions! I went on my two week trip, and occasionally got it right thanks to the new ideas and having my CGM safety blanket. It was really exciting to eat a meal and see a low, smooth “hump” on the CGM, coming back down to pre-meal numbers after 2.5 hours, without a crazy high spike at 1 hour.

Not saying I get it right every time, but I was pretty happy with how it went considering it was 100% restaurant food, with wild guesses for carb counts. :smiley:

For now I’m focusing on getting my initial carb bolus right, and not worrying too much about the protein after-hump. Baby steps!

I think it’s possible your basal rate is too low. Four to five hours after a meal, it’s tricky to disentangle the delayed effects of a meal from some separate rise in basal insulin requirement.

I personally will correct something if it’s just an hour after a meal IF it’s something my son has eaten a lot and I know that 60-minute number is higher than usual. So for instance, he typically spikes to 160-170 about 60 minutes after eating oatmeal. If he’s at 190 or 200, I’ll give him 0.1 or 0.2 units of insulin, because I know his habitual response and know he’s higher than he would be, which suggests to me that it will take more for him to come down after a meal.

It’s helpful to have a notebook and record your hour-by-hour response to a meal, with BG, insulin dosing, etc.

Yeah, you may be right. I’m flying on my own figuring out a basal. After months of conservatively upping my basal in fear of lows, now I have a CGM, I’ve finally gotten my overnight BG to stay a steady 100-120 all night. I dislike fasting in the evening, so I’ve been avoiding doing an evening basal test but I’ll do that this week. :smiley:

Yes, but that guy – nobody – has the most incredibly flat BG among diabetics.
