Basal program physiology

Hi all, we’ve been adjusting basals for Kennedy and weRe not quite there yet.

I have read tht everyone is different, but that usually only one peak and one valley is necessary for basal regimen. We did tweak up from 3 am to 7 am due to emerging morning highs commencing after 3 am

If you are seeing highs a typical time of day, how many hours before do you make the basal increase?

I use humalog and my insulin duration is 4 hours. If my bg was consistently too high from 3 am to 7 am, I would increase my basal rate from 1 am to 6 am and see how that went. YDMV.

The hardest part about basal rates is that you will lock it in and it will be amazing for 2 weeks and then the seasons will change or stressors will enter life and it’s back to shooting at a moving target. Good luck!

For my son (10 yrs), we bring the basal up about an hour before his numbers start to rise and stop about an hour before he starts to level out. Having said that, I don’t think it is an ‘exact science.’

I recommend that book “Pumping Insulin” by John Walsh. He explains the timing of basal rate changes, but, yes, an hour or two before a change in blood sugar should be enough of a headstart for a basal rate change to start to take effect.
