Basal Ratio

Im on levemir right now and should have my pump supplies this afternoon… but what im wondering is how you go about transferring the levemir dosage to basal… ive been on the pump and my basal was i believe 1.35… now im taking 34 units levemir… sooo i did 34 divided by 24 = 1.41 units per hour… can that be all there is to it? hmm i wonder please share your experiences and or thoughts on this! thanks everyone

It sounds like you have a starting point, ie; you were on a pump before. I it seems pretty close but I would refer to “Pumping Insulin” the book by John Walsh for setting up your basal rates. Unless you are seeing a CDE or Endo office, I would let them do the math for you.

yea i agree but i was just wondering cause i was like let me do simple math… and it came out quite close to what i was on before so i was curious if its really that easy! you know that would be crazy that is something a 14 year old can figure out…

I think the continuous trickle of the pump is more efficient and don’t need as much as on MDI. If you remember the 1.35 correctly then maybe do that, otherwise play it safe by taking 80-90% of your MDI amount and then increasing it as needed if you run high. So maybe try 1.41 x 90%=1.27 per hour?

yea that is true… i guess i will be fasting! lol but its worth it…

just a update for those who care… ive pretty much ironed out the basal issue… its 1 unit flat because ive been taking vitamin d… so insulin resistance has been decreased
