Combo Boluses

I have a CDE friend who suggested you gotta eat an awful lot of steak to consider bolusing for the steak. If I bolused for protein, I would go low. Plus I dont eat enough protein to even consider it.
I know I have a problem because one unit can often drop my BG 100 points, so insulin sensitivity plays in there but a hamburger and fries at the local burger place, is perfectly taken care of with 4.5 units.
Everyone has factors which are totally unique to them…sensitivity to insulin, slow digestion, etc. and I think those things are more important to work on and consider. They also have a lot to do with the results you might get using TAG or any other technique.

Just spoke with my CDE today about TAG. She said Johns Hopkins says that if you eat 5 oz or less of protein, there’s no need for an extra bolus. Each ounce over 5 should be counted as if it were 15g of carbs. She also said that Joplin believes that unless you’re eating a lot of fat, like the proverbial pizza, they see no need to add insulin for fat. Haven’t had the energy to look for the studies yet, so this is anecdotal, albeit from a very trusted source :wink: I’ll look tomorrow… maybe.

No problem - everyone has to find teir own answers. My goal is to manage my diabetes versus the other way around - lol

That’s a good point, Laura. I think, especially online, it’s easy to see only a small part of each of our diabetic pictures which consist of many factors, some of which we aren’t even aware of that may make one person struggle and one person have fairly effortless stability. Then there are the choices we all make that determine our quality of life, balancing those values against A1C’s.

I have only been on a pump for a month but have learned what works for me. Depending on my bg, I give myself between 25 and 40% up front and the remainder over 3 hrs; remembering that at 3 hrs I still have 4 to 5 more hours that the insulin will be working to lower my bg. YMMV

Thanks, Marty. Good work on figuring it out in so little time. I’ve been on my Ping for three months and in the first month I was still working on basal adjustment and getting a set to stay in right! I hadn’t really thought about it that the insulin keeps working beyond where you extend it. How much over your normal total bolus do you do, or does it work for you at the same amount?

I did another relatively high carb/fat meal last night (this is my week for problematic groceries, I guess!). I did a 50/50 split and a total of 2.75 for what would normally be 2.10 and spread it out for 4.5 hours. Before dinner I was 103, then at the 2 hour mark I was down to 66 and at bedtime up to 146. This was a lot better result than my previous stab in the dark attempts! So tonight with more of the pizza I’m going to try 40/60 and do 3.25 for a 2.38 total. Since I only eat one piece, the recipe makes 11 servings so if I can’t get it right with this experimenting it’s too unstable to think about…at least for another 10 months!

My experiment is going well! Last night I bolused 3.25 (instead of the 2.38 my ratio called for). I did a 40/60 split extended out for 4.5 hours. I started at 75, was 91 after 2 hours and 72 at bedtime! Yay! Considering the first couple days of my experiment I was in the 200s at bedtime! If I start that low I might actually cut the dose back a little, though when you think about it coming back down to where you started by bedtime and not spiking in between is perfect!

thats what I do and was told to do depending on meal, half and 1/2 , seperate by time do you want 50 50 or 60 40 ,45, 55 ?? ect?

usualy a half hr is good but you decide and on what meals
