New guy needs help

I have Gasteo also and it is not consistent. There are days where my BS goes up the usual 45-60 minutes later and I have days where it takes 4 hours. I have learned to not take my shot until I see my BS go up especially since if I have a full on Gasteo. episode my BS plummets.

You be careful with that pump, @Rleb. Your not doing that bad. Don’t make anything worse. Some of those bolus techniques are like mainlining insulin. It can take you out. I’m a big fan of extended bolus, but I tend to overdose as opposed to under-dosing and if I’m off at all, it knocks me on my ■■■. Extended bolus is a powerful force. Use it carefully at first.

@RobinLea, is it ok to set the basal in a typical way when someone has gasteo?

That is what I do but I have never had my Endocrinologist or Gastroenterologist say anything about it.

If he is using a pump, we should probably change that purple Lantus column to reflect his pump settings. What pump model are you using, @Rleb ? What are your current basal settings?

I thought you were Lantus manual injection. Let’s talk about this basal stuff before you make any changes.

We need more data from you. You are higher risk because:

1.) Your have new tech
2.) Your new to basal adjustments
3.) You have gasteo issues

Your data isn’t perfect, but its ok for a new pump user.

I am using the Medtronic 670g. My Basel rate is 0.8 all day right now. I have the Medtronic cgm but it is not connected to the pump its the gradian connect. I had it prior to the pump. they want me eventually to go to the sensor that communicates with the pump but I don’t really trust my cgm I have found it to be off quite frequently. now in the last few days with low carb tho it has been very close and sometimes the same. I guess less variation. I also don’t know how bad my gastro is but I’m not at a point where I would want to wait to see a rise. its also very difficult because when I’m eating a normal diet I have a rise after I eat but it will not curate with what I eat at that meal if that makes senes. when I took the emptying test. they said it was 100% at one hour but I saw a rise after eating the bread and jelly that morning. those familiar with the test might remember your given eggs with the tracer, and two slices if toast and jelly. when I ate after I ate I seen a large rise with the first hour. when the result came back it said nothing emptied until the second hour. I was confused but it made me thing that possibly food stays in my stomach from the prior meal until I eat again. now I could be completely imaging that and I just have no idea how carbs and different food enter my system and when and how my Humalog works either. I just always assumed things should/did work faster. these are things id hope my endo would be able to help me with but now I’m seeing she isn’t really able to. that’s why I’m seeing a new one. ill post some more data an some more cgm graphs. you can tell from the excel file when I eat and when I bolus and with the extended bolus.

With this 670g, we aren’t dong anything closed loop, correct? Because there is no CGM connected? I just wanna be sure.

I think that you talk to this doc about bumping up your daytime basals so you aren’t needing correction during the day. That might be our challenge to him - to see if he can come close to hitting the mark on that dosage calculation. By then, you might have been on the pump long enough that they will agree to fine tune these dosages.

I don’t think that you should do it all on your own the first time. What if you made a mistake? I mean with the time interval settings and stuff. If you have never set that up before, it would be nice if he setup some various time intervals where you could play with the dosages.

Yes you are Correct. No looping. I meet with the doctor tomorrow. Im hesitant to up the Basel now with this low carb my numbers are a lot lower and tend to even go low sometimes. I’m trying to figure out a day to fast for a couple meals any maybe check my Basel rate. I’m still confused on how long to not eat to check the Basel because I don’t know how long the food is sticking around. hopefully this endo can help with some of that. ill post some more of the updated data and see if you guys have any thoughts. its been a couple days of low carb and altho its difficult I think I’m feeling better and blood sugars are some what stable. I know the extended bolus is dangerous but lucky right now I can really take close care of my self and check my sugars when ever. I’m not doing much else. Im new to all this so I’m slowly trying to learn.

mmmm…good points.
It might be nice if he sets up some different time intervals for you so that you can raise or lower the basal as needed. Ask him to do that for ya’, so you don’t have to do it yourself.

I like the book “Pumping Insulin”. It gives pretty much a step-by-step on figuring out your insulin needs, both basal and bolus.
