Unfortunately I just used the last 6 units of Lantus out of my usual 18 units basal. Went on an overnight trip and forgot to bring a spare.
Any advice how I can/should compensate using Humalog tomorrow until I get back home Tues evening? I haven’t had to deal with a 1/3 basal dose before, not sure what to expect.
Depending where you are, I believe most pharmacies have to give you insulin for emergencies. I believe you are allotted a specific number of these incidences per year.
Just to follow up with how things worked out…overnight on the 1/3 basal dose I noted nothing unusual (BG 100-110 overnight, woke up at BG 102). During the day Tuesday I used 1 unit Humalog every 2 hours as a pre-emptive correction and upped my I:C for meals bolus slightly and it seemed to work out great with good BG’s all day. Tried to be my own manually operated pump. Tuesday evening I got back home and back on my normal full Lantus MDI basal and stopped the pre-emptive Humalog corrections. Overnight I stayed at 150-160 all night long. This morning got back on track starting with breakfast bolus.
My summary: Monday evening basal was 1/3 of normal but fasting BG was OK overnight and waking…daytime Tuesday was fine using 1 unit Humalog at 2 hr intervals to replace missing Lantus basal…Tuesday evening basal was back to 100% but BG was medium-high all night.
This unplanned experiment seems to be telling me the Lantus basal is working for more than 24 hours for me, probably closer to 36 hours. Not sure if that’s valid but I have all my CGM plots to go over with a CDE and figure it out.
