A1C <5 - MDI & No CGM - Bernstein Tweaked

Fairly strict? I’ve taken Dr. Bernstein’s recommendations and applied some tweaks. Some examples:

Carb Counts: I deduct 100% of insoluble fiber and 50% soluble fiber. FMI:
Fiber Content of Foods in Common Portions; HUHS (Harvard University Health Services)

Bolus: I use Apidra twice per meal/snack. Once for carbs and once for protein (I TAG 60% for protein but don’t bother with fat as I work it into my basal). I also stack.

Corrections: IM w/ heat and exercise – e.g. quad, warmed rice pack + squats.

Note: Dr. Bernstein initially recommends 6-12-12. However, he allows more carbs as long as one doesn’t exceed +/-10 excursions with meals. He also allows snacks following the same rules. Unlike many PWDs who find Dr. Bernstein inflexible (See any thread re: Bernstein + @Jag1 for some sterling examples…), I find him quite reasonable. He has one patient that uses chocolate chip cookies as excarbs. Another uses peanut butter and jelly sandwiches!

My TAG for most days is ~60. I don’t monitor ketones unless I have a reason to be concerned, but my docs test me regularly. I always test positive. Generally, between 1-3. After a good workout, I occasionally will go up to ~5.

Do you mind sharing what needle length you use for quad IM? So far all correction injections for my son have been subcutaneous (at least intentionally - he is so skinny we probably have gone IM a few times).

I’m fairly lean (though not skinny!), I just use my usual 8 mm. Delt IM w/ push-aways in a hot shower works fast, too… :wink:
