Hypo-Phobia?

I’m a little confused, Riz. You said the Lantus works well for you for the day and is too strong at night. But then you said you went to bed at 15 (270) and woke up at 7 (126). Are you saying you think 126 is too low? I think the reverse! Your bedtime number is too high and your wake up number is ok. Maybe I’m misunderstanding.

I do agree with what the others say, that splitting Lantus into two doses or else switching to Levemir is better.

If I went to bed at 15 and woke up at 7, yes Lantus is too strong no ? The drop is 8. My idea is if I went to bed ok ( 7 ) I will drop dramatically.

I see what you are saying now. Yes, it is a big drop. But I’m wondering why you were 270 at bedtime? Perhaps your carbs at dinner made you high and you didn’t bolus enough? So your body is just regulating to a good number. If you go to bed at 7 (126) that doesn’t necessarily mean you will drop the same amount and be too low. But we all have different basal needs at different times. (That’s why the pump works so well! I have 6 different basal rates!) I would talk with your doctor about changing when you take it, splitting your dose or switching to Levemir which is more even in its action.

No pumps are available in Montreal? Have you spoken to other Canadians on the board? I know there are many on the pump, but I also understand that each Province has its own regulation. That is terrible!

PS Pump are covered by the Province for adults in Ontario and Yukon …and if you are covered by Insurance through your Employer , private. So I am all for Advocacy !

How much insulin do you take with meals? Does your mealtime insulin make up around 50% of your total daily dose? If it doesn’t you may be able to get better control by reducing your basal and increasing your bolus insulin.

In the Quebec, the pump are not covered… Im an " artist " so i’m on my own. I have to wait until this is covered by the government. Really not cool…

During one of my forays into hypo-dom, I lit a 1/2 gallon of ice cream on fire b/c I was defrosting it under a broiler. Ooops. I have had some hair-raising experiences and it’s no fun. I am still waaaay more paranoid about complications.

Hee hee, unfortunately there was also cursing @ a wino in French when I woke up in the ER, pounding my head really hard on a concrete floor, driving a car over the ground floor of a parking garage, the IV came out and the blood smelled like meat. All kinds of horrors. I still have my feet, sight and kidneys.

As I mentioned in an earlier post, I am new at this (as a mom of t1d), but I am also a mathematician by training, and I have a problem with the concept of a linear formula like “give 45% long-acting” (or some such target for Lantus as proportion of total dose). Where is the research showing that formula works at the extreme end of minimal insulin need, early stage type 1 or LADA? It stands to reason someone who is still making their own insulin (whether due to LADA or honeymoon or what have you) who is on an extreme low end of insulin use is not necessarily going to be as well served by a standard linear formula like “45% long acting” that might apply well to someone with long term t1d. My son mainly needs a little help when he consumes a carby meal (like the gluten free mac and cheese he had as a main course tonight). If we give nearly as much Lantus to match the novolog for what he ate on a typical day (when he probably overindulged in carbs, he is a kid after all) he is going to go hypo at random times because of the Lantus. His novolog dose is anything from 4 units to 8 units per day depending on what he eats (an ice cream and cake dessert such as he had on his birthday pushes him to 8 novolog total for that day). Even 2 units of Lantus seems too much lately. More fundamentally, formulas are nice but where is the research to support the idea that the ratio should always adhere to that linear formula (give almost as much Lantus as short acting insulin), at all stages of the disease, and for people of different ages and weights? My guess is the research support isn’t there. I’d be interested in what other people think about those kinds of formulas.
