Does anyone else ever have issue where the infusion sit works when you bolus but does not really work well for basal? We always change my daughters site at dinner time that way we give her 4 or 5 units and know it went in. The bolus worked fine and her BG came down as normal but she woke up this morning at 300. So we gave a correction with breakfast. At lunch she still was around 200 so we corrected again. In the afternoon I checked her 2 hours after lunch and she was again 230 so I had her correct and when she did that the pump alarmed. Very frustrating.
Almost sounds like a bad site. I have had a problem before with a bad site to where the insulin is infusing between the fat layer and skin layer. It does not absorb well, but it may appear like it is bolusing well. BG may come down some, but usually the improvement you see is very minimal. The cannula may have been kinda positional where it wasnt completly blocked but over time it became that way. I would have changed the site out had I not seen significant improvement after correcting, especially if having corrected a couple of times with minimal improvement.
Also with Basal you wont necessarily get an alarm for no delivery, it is such a small amount infused that is why it doesnt trigger the alarms until you go to bolus. The larger amounts cause more pressure to back up and trigger the alarm. Has happened to me on occasion where first bolus seems to work, but over time and additional boluses the pressure level builds up and you get a no delivery alarm.
I tend to agree with Christy that perhaps the site you're using is just not absorbing well. I've found that in some locations I hardly get any absorbtion, whereas others it actually absorbs a little quicker.
I've just learnt what sites work better than others and go from there. Then I adjust my rates according to the site I'm using so that I can still rotate around.
