Tresiba Basal Insulin

I do my Tresiba at night before bed, just like my lantus. I usually have a before bed snack, but my levels are usually the sme in the morning as what I went to bed with. what time are you taking your short acting ?

thanks Rozale. What do you mean what time am I taking short acting? I’m using Tresiba for basal and Afrezza for bolus. That’s all.

Short acting b4 meal. I am still terrible at prebolusing, but so far not too bad results. I migh have figured it out now. Go even lower with tresiba (attempting 13 tonight but increae I:C ratio up to 1.5 u/10g carbs.

Does you pump allow you to set your I:C ratio at anything other than 1:X? A 1.5:10 ratio is equivalent to 1:6.7. My Animas Ping does not allow tenths of carbs and I wish it did. What pump do you use?

Well i am on pen now so no bolus wizard. Plus, i never used it anyways, just calculated in my head. However, medtronic pumps allowed it i think. You could choose how you calculate carbs, in grams or in units (BE, calculated unit, no idea how you call that in english). If you were on that, you could tell the pump how many units you take for a carbunit, and that carbunit could be whatever you wanted.
It just asked you for the units you are eating and told u how much insulin you needed. I think though that omnipod doesnt have that option. Plus it was somewhat inaccurate as only half carb units were possible

Right on Mike. Do you actually carb count with afrezza these days or pretty much just wing it? I still use novolog or combos in certain situations but with afrezza I really don’t bother to carb count.

Do you have plenty of 4s on hand until u get the new basal figured?

Tresiba has been available since early december. I guess they made it official today

I am sure by now he would think carb counting ( atleast for PWD on Afrezza to some extent ) is so medieval thing to do .

That’s been my biggest beef with the Vibe (and 'Pod) – no fractional units for ratios, etc. I WANT SNAP BACK :frowning:

Hey Sam. Yes plenty of 4’s thanks. I figure it will take a couple days to regulate the Tresiba and then I’ll be good to go. Thoughts on that? I feel like I need to fast for a whole day to see what the Tresiba is doing LOL. So far the afternoon was on the high side but it’s just so easy to correct with the Afrezza.

As far as the carb counting with the Afrezza goes . . . well let’s say I don’t worry about it much. If my carbs are going to be 60g or more I use an 8u. Anything under 60 I use a 4u.

Well it was more simple for me not transitioning from a pump but I think if you started with less than you’d been using with the pump and are seeing high trends, there’s probably no harm in adding up to the level your pump was giving you if you wanted to- but id let it sit there for a few days to balance out— you definitely don’t want to get into a pattern of tail chasing by trying to adjust every day an insulin that lasts multiple days…

agree. I’m gonna give it a few days and go from there.

I just use the bolus wizard to get me close and then dial up an increased/decreased dose to simulate what I want. These are computers for gosh-sakes. Why they don’t design that feature into the pump baffles me. I would have loved to been at the table when that decision was made!

You could just adjust your carb count. If you need a 1:1.67 and all you can get is a 1:2 then you could just adjust your carb count by reducing it 17% (1- (1.67/2))…

First day not bad but clearly a bit more basal. Going to up my dose and see how it goes. Overnight was relatively steady which was a good sign. Went up a bit.

Today has been excellent so far. I fasted until lunch and had a straight line. So far so good and no pump attached!!!
