How do low carbers bolus for their higher fat and protein meals?

I’ve noticed that when I eat meat and even a chicken ceasar salad with enough dressing to actually make it taste good, the high fat and protein will cause me to go high after several hours. I haven’t figured out how to bolus for that. My pump trainer says that if I do a square wave and the readings start going up about three hours after a meal, I should have my square set for one hour as it takes about two hours for the insulin to be active. That didn’t make sense to me and setting the square for an hour didn’t work for me anyway. So, what do you all do?
