The issue with Metformin is that in patients with chronic kidney disease may not clear the drug enough resulting in elevating levels of Metformin which can cause ketoacidosis.
I would think that this would have to be looked at against the kidney function tests that are routine for me. I think the issue was with people with a eGFR of <30 which is stage 4 of 5 of chronic kidney disease.
I have been on a low dose of Lisinopril, an ACE inhibitor, that is shown to have a protective effect on the kidneys for years, even though I have never been hypertensive. My doctor prescribed it because kidney failure is one of the common diabetic complications
