Honesty, while on vacation, I stay in my old time zone the first day and make all my changes to meter and pump that night as I drift off to sleep, dreaming about the beach and the sun awaiting me the next day. :slight_smile:

I would recommend to inject the splitted dose too. Not all Lantus users get 24 hours and this means there will be a gap in basal converage. In many cases this can be covered with a higher dosage of fast insulin at the time where the gap occurs. But this can be troubling if you decide to skip this meal for some reason.

The translation to the splitted dose will be bumpy: you need to increase your test frequency to correct your blood glucose. But after the second injection of the Lantus you will have a more robust basal coverage. The 12 hour scheme is less convenient but the progress is worth a try.
