The goal of this survey is to study participation on TuDiabetes so that we may help improve the design of this and other online health communities.
Participation in this research project is completely voluntary. You have the right to say no. You may change your mind at any time and withdraw. You may choose to stop participating at any time.
Upon completion of the survey, you will be entered into a drawing for a $100 Amazon Gift Cards, or you may choose to donate the $100 to a charity of your choice.
