Target bg

I started on my pump today and my doctor said that now my target bg for the day is 90-100 and for at night it is 100-120. Do you think that this range is too strict?

I’m not a pumper, and you will get responses from some I’m sure. But imho those targets are highly unlikely for most Type 1’s! I don’t think there are too many of us who can keep our blood sugar in that type a range. I also question what your doctor understands about your diabetes management by giving you a blanket target for " day" and a blanket target for " night". Is he a PCP? They just don’t really know that much about insulin use! Your fasting blood sugar (which is ideally under 100-though we are all not perfect at attaining that!) is one thing. Your post prandial is another. Some people do try and get under 120 two hours after meals, but many of us are happy under 140. I myself prefer to be lower than that before meals so I have room to grow so to speak.

While I don’t think it is bad to have such perfect blood sugar targets as an ideal,I think it can be discouraging if we take them as more of a mandate and then are upset when we exceed (or are lower) than those very strict numbers. I think it is a balance between having good expectations and setting yourself up for feelings of disappointment and failure.

No, because odds are good these are just settings for the bolus calculator to use, it’s not an absolute “OMG you have to be in this range all the time”

I actually have my pump set to 80-100 during the day and 90-110 at night… am I always in those ranges? No, but the pump corrects me to the high end of those ranges when I’m not.

I think that’s a good target. I’m not saying I could even hit it one fifth of the time, but a good target.

Some people come here and complain when once a week they miss their target, and get a 112, like it’s the end of the world. I kinda contrast that with what happened to me this morning - woke up with a 342 at 4AM for completely unknown reasons - and it’s like we’re on different planets.

Who is your endocrinologist? Nurse Ratchett? Talk about no fruit cup! Those are some stringent ranges and not at all realistic for an insulin dependent T1. My target range is 80-150 with no real difference for night as compared to day. Even with my pump (and I’m pretty new at it too) my fasting is higher than the target. I’m thinking I need to set a temporary basal from 2 to 6.

Do you see an endocrinologist of GP/internist? Those ranges sound like they come out of a “Diabetes for Dummies” kind of book. Cleaving to these targets is going to be a frustrating and difficult proposition. As a T1, you will spike to 180 or more with a meal.

Absolutely I do… I started w/ 90-120 and saw a ton of readings on all my reports “out of range”, made me feel like I was out of control and failing. I changed my range to 80-130, and it helped my self-esteem on my pumping “success” immensely

That’s pretty close my ranges. Once you get all of your ratios and rates figured out, your pump will keep you in that range pretty well.

Those are goals, so don’t feel overly pressured. Did your doctor mean 90-100 between meals? I’m on MDI & my targets set by endo are 90 between meals & under 100 fasting.

My ideal BG is 100 during the day and 120 at night. That seems just in the range of perfect for use with the pump. Since you have so much flexibility with the pump, that shouldn’t be too hard to attain over time, once you get the hang of everything. I have my High BG threshhold at 140 and low BG at 80. What pump did you get?

I think, this is what Doc is telling you : I am a pumper and agree with Sarah …I don’t know, what type of pump you use ; this is how it is for me on the Medtronic 522 : bolus wizard set up …I have my setting set at " BG target 5.5 -5.5 ( x 18 , I live in Canada ) …the correction factor on the pump keeps this in mind …and when I am lower the pump tells me how much I need to treat, when I am higher the pump tells me , by how much insulin to correct. As one gets more familiar with one’s process as we are all different, one can adjust . If you feel uncomfortable with the numbers …try them out and adjust , after you have checked with your pump trainer .And believe me , the target is just a target , that you fed into the pump …

really… high and low set the same? interesting, I thought the bolus wizard was designed to bring you back into the middle of your range (on minimed) so for a 90-120 it would try to correct you to 105

I’m using the dexcom sensor now… really wish we had all devices that were capable of talking to each other

And Nel is dead on when it is “just a target”. We are human, none of us perfect, and sometimes things just won’t work every single day. Don’t pay as much attention to what the numbers all, but look at the pattern presented by your test results

I suspect you will do just fine, but if you have questions… I think there JUST MAY be a few opinionated folks here who can share their experiences

OK for a target, but nearly impossible for a T1D to achieve consistently 24/7.

This range is GREAT! It’s your TARGET! And as your pump uses the information, you will find yourself HAPPY! Not too strict at all.

Hmmmm…after reading this I believe I need to question my Endo. He has my target set on my pump at 125 for the high AND the low. Do any of you have only ONE target set on your pump?

Very nice! I love mine! I think once you get in the swing of things, and know how your body reacts to food, insulin, rest, etc, then you will be able to get in the ranges your doc is suggesting. Please keep us updated as to how its going.

DO you have medtronic paradigm pump - should have looked on your profile. I cannot believe I do not know where to find what my target set is at for my pump - I have good control but need to know this number. Under what heading do you normally find this on your pump? I am scrolling around and cannot find mine. I cannot be bothered going through the manual - I will if I dont get a response though

Target range for blood sugar and on the pump mean slightly different things. My pump target range is 90-120 right now, but I have it had 90-100 at times. All it means is that it will automatically help you to bolus if you are over that range. So if I am at 140, when I enter that it my pump it will bolus me for the amount I am over that range. There is not an expectation that every reading will be in that range.

Ahh… that little fact brings somethings to light. My corrections will usually bring me into range which indicates that my insulin sensitivity may be set too low
