Type 2 and very sick; what do i do?

I have been sick since last week thrusday, but since Sunday morning have not kept a single thing in me not even ice chips. Not able to take insulin or my januvia in effort to not drop big time right now. My Dr sent me Weds at 11am for 4 hours of iv fluids and compazine by iv at the ACU (outpatient procedures and surgerys) and by 8pm i was in the ER sicker than ever, they did a ton of tests and cant figure out what is going on, called my Dr thursday morning, she cant see me until tuesday because she had dialysis patients all day, and told me to get right back to the ER because there has been zero improvement. They did a bag of fluids and then tell me there is absolutely nothing wrong with me and i shouldn’t be there. My white counts rising fast but nothing wrong with me? Plus i cant even handle ice chips?? My Dr is not in on fridays but the Dr shes in with is, i called talked to the nurse who talked to the other Dr they said i need to go back to the ER (havent yet) I am afraid to because i am being treated like i should not be there, but i havent kept anything in me in almost a week! Sugars are going from 70 to 130, all over up and down. At least im not real low or high. Checking about 6 times a day instead of my normal 4-5 more often 4. How do you handle diabetes when sick like this? Do i keep pushing the ER to help me?

I would. Something is going on which is probably NOT related to your T2.

And not keeping anything down would be reason for ANYONE to continue getting more help (IVs, tests) in Urgent Care or ER.

Have you checked for ketones? Unlikely with the good BG range you’ve noted, but worth checking.

Didn’t they send you home with an antiemetic like Zofran (Ondansetron) from the ER? If not, you should have your primary care physician call in a prescription to your pharmacy.

They checked for ketones. I have phenergen pills i can not keep down and suppostories that dont do a thing. They put a anti nausea patch behind my ear it didnt help at all. They dont even have me try to drink water before sending me home. I am freezing or sweating no in between and i know ill be going back today. I feel so weak! Trying to figure out why my sugar goes up when i get sick? Shouldnt it go down? I will update when i know anything.

Your BG goes up likely because your liver is on overdrive releasing glucagon to maintain your blood sugar in the absence of exogenous carbs.

You need Zofran ODT (oral disintegrating tablets). No water needed, and nothing to keep down; just place one under your tongue and it’s gone in a minute or so.

I have to tell you, it is your doctor that is letting you down with the follow through. The ER will simply stabilize you. They likely didn’t tell you that nothing was wrong, only that you should follow up with your doctor to figure out what is going on and get appropriate treatment.

And while you might think you aren’t keeping anything down if you keep at it you can get some hydration going. Don’t forget you also need electrolytes.

As to this weekend, if your doctor won’t see you can also seek out the ACU or urgent care. They might be able to give you something stronger for anti nausea and they can perhaps do a little more digging to see what is going on.

Go to the ER and ask for the Zofran ODT immediately.

The ODT, as Dr Rose says, is an orally disintegrating tablet.

It dissolves in your mouth and gets absorbed immediately

You will get some to take home with you, or get a prescription for them.

An anti-emetic stops nausea and throwing up. Stops it right away. I have seen this work with my daughter. And @rgcainmd knows her stuff.

Go get this right away, today, “STAT”!
(That’s nurse talk for “do not delay!”)

Once your body is no longer wretching, everything else can be addressed.

And yes, it is Very Common for your blood sugars to go quite high when sick.
When you are dealing with extra medical issues, the glucose control can go awry.

You seem to be ok at the levels you reported (70-130)-- for now.

If it goes way up, report that to the ER doctors as well.

Brian actually yes i was told there was nothing wrong with me by the ER Dr on Thursday night. My mom heard her say it too. Our ER/Urgant care is is the same place, they triage you and put you where they think you need to be, i keep ending up at ER. I am a HORRIBLE poke on a good day, when im sick? UGH Took 3 nurses an hour and half to get an iv in me on Weds and not much shorter in time Thursday. I am dehydrating fast i was told by the nurses.

LADA lady, will that help more than the compazine and zofran i been given by IV? Becuase that plus the patch behind my ear, i STILL throw up. I will ask them about it, mom said were going in soon because i am throwing up when i havent even tried ice today yet. And i dont have zofran but i have supposotories of phenergen (stronger than zofran) and does nothign for me.A little back ground is, when i get sick i get very very sick very fast, and this has been over a week now. i have had 5 hospitals stays in 3 1/2 years, i have fragile health on a good day, thats why i dont know why they are messing around!

Zofran (in an adequate dose) is much more effective than phenergan or compazine for nausea and vomiting for the vast majority of people. It is used to treat nausea in patients receiving chemo.

Additionally, Zofran doesn’t carry the same potential risk of causing the very unpleasant side effects of acute dystonia or akathisia as phenergan and compazine.

I was given iv fluids, zofran by IV, compazine by iv and protonix by iv, told there is nothing wrong with me, on my way out asked for my test results, i have a major UTI (i get them a lot so know how to read the tests my dr taught me) and my white counts over 18,000, plus other tests are off. I have lost 10.8 lbs in 4 days, but theres nothing wrong with me???

So you went to the ER. You saw your tests and believe you have a serious UTI. I presume they did do a specific test for a UTI and they then sent you on your way? Did you tell them you thought you have a UTI and that it could be serious? Did they not think you had an infection. An untreated UTI can really become a life threatening condition.

From my limited knowledge, nothing they gave you treats a UTI. You likely need an antibiotic. I would go to see a doctor, be insistent that you have a UTI and it is “serious.” And guide them to a proper treatment.

I am seeing my Dr on tuesday, and yes i know how serious they are, my husband died from sepsis caused by a UTI. They did do tests for them, i have chronic UTIs. This dr is incompetent others have issues with him too, as soon as he walked in my mom cringed when she saw who i had. He even told me i have a bad Dr if shes sending me to the ER and not getting me in the day i call, i told him, very politely, that she has more than just me for a patient, and by sending me to the ER she is trying to help me. If i am not better tuesday i will be admitted. Even after all the meds i am still very sick… My BG is holding around 118 with out insulin or januvia in 8 days, since i cant eat or drink.

I just have no patience for a doctor like this. Unless you are forced to use this doctor I’d recommend you try to get a different doctor, one who can actually help you.

This was the ER dr, MY dr is great, shes internal medicine and takes 45 mins to 1 1/2 hours with me each visit. I am so frustrated and so sick, and how can a Dr in an ER tell you that if you come back he wont treat you? Esepcially when im so very dehydrated!

I guess I am confused. You have been to your regular doctor, the ACU and the ER. Did any of them attempt to diagnose you with a UTI? This should be the sort of thing that you could get help from at any of these places. In my area there are also urgent care centers that you can go to.

Same here: confused. Diagnosing (and treating) a UTI isn’t rocket science. Which labs (name of test, result, normal range) are leading you to believe you have a urinary tract infection?

I found BIG problems with waking up throwing up. found out I was having an allergic reaction to the insulin. dr. and I had to experiment with it to figure out a way to take it so it wasn’t such a blast to my system . I take Lantus 2x daily. usually it’s taken once a day to cover a 24hr. period. but the amount was too strong for my tummy. so we split it. I do 10 units in the morning with breakfast, sometimes I have to eat some cheese or cottage cheese first to settle my tummy then do the morning 10units. eat through the day then do a second 10 units after dinner with an evening snack, milk or cereal. make sure you at getting enough fat in your diet. it helps coat your tummy. I couldn’t take the Humalog at all. I just barfed and barfed. it is just way too strong for me. the doctors think it should be o.k. for everyone but that is not the case. follow your feelings in the morning,what sounds good. if nothing start with milk or chicken soup, no meat or noodles. Knorr makes a natural and less salty version. or make a beef broth. try a half a banana anything that you think your tummy can handle. I started with saltine crackers and used cottage cheese as a fat. it takes time to get your body used to this crap but just keep looking. i’m always around if you have any other questions. it’s taken 10 years for me to find a good balance because that insulin is VERY STRONG on a tummy that didn’t even like aspirin or Tylenol. My prayers are with you.

Sorry so long to get back to you.
So you had the zofran in the ER.

But you need it CONTINUOUSLY
In the ODT form
So you can take it round the clock when you are not in the ER.

An IV dose of zofran only lasts a certain amount of time.

If the problem causing the nausea has not been solved, then you need the zofran ODT to stop the nausea when you are NOT in the ER – ALSO!

Now
Go to the good doctor tomorrow
Get the zofran ODT (or prescription)
Get the antibiotics you need (prescription)
Tell the good doctor how the ER doctor mistreated you
And start drinking LOTS of water, as soon as the nausea goes away.

Or find a different ER, or ER doctor. It might also make you feel better to file a complaint with the ER about the bad doctor.

Consider requesting domperidone prescription from your doctor. I have type 2 diabetes and am nauscious frequently and take 2 - 10 mg tablets of domperidone and it helps. You shouldn’t have to suffer until you discern what is causing the nausea. I had the exact same experience at the ER, in fact was bullied there by the nurses and told by a doctor I shouldn’t be there. I too am reluctant to go back there which isn’t right because there is a chance I will need to go there. My kidneys and pancreas and liver may be diseased due to the diabetes not being detected for maybe 11 years. This is being investigated by a doctor of internal medicine. Ask for a referral to one. I also have high blood pressure and was told I’m at risk for a stroke which may require visits to the ER. We have to understand the ER is a triage for ppl in emergency health care situations. Go to your general practitioner, ask for referrals and be patient. FIRST get something for the nausea and relax through deep breathing exercises. Concentrate on diet and exercise, the key ! ! I found out September 2, 2016 and have been on a strict paleo/Mediterranean diet and walk, jog 8 blocks 1 - 2 times a day. Must be proactive, there is no panacea from the doctor. We have to take charge. Read Dr. Mercola he was diabetic and reversed it, lots of good researched information. Wishing you the best ! ! I understand how you feel. :raising_hand_woman:

I have now read your doctor is an internal medicine doctor and you were give medication for nausea. It sounds to me, as well, you do/did need to be at the emergency re. possible dehydration and urinary tract infection. I agree with the friend who said you need to make a formal complaint about your ER experience. SO WRONG OF THE DOCTOR TO HAVE DAID THERE IS NOTHING WRONG WITH YOU. It is true, we have to accept, that doctors only know a limited amount of information, some are better than others. So I agree you need a second opinion, doctor.
